Introduction

A lymphatic malformation is a congenital abnormality of the lymphatic system caused by improperly developed lymphatic channels. Lymphatic malformation treatment depends on the type, size, location and symptoms of the lesion. Small, stable and minimally symptomatic lesions may be observed, while lesions causing swelling, pain, infection, leakage or functional problems may require active treatment.

For many suitable symptomatic lesions, image-guided sclerotherapy is an important minimally invasive treatment option. Surgery may be considered for selected localized lesions or residual disease. The aim is to reduce the abnormal lymphatic spaces and improve symptoms, function and appearance.

Key Points

  • Lymphatic malformations are usually developmental and present from birth.

  • They may be macrocystic, microcystic or mixed.

  • Ultrasound and MRI help determine their size and extent.

  • Sclerotherapy is commonly used for suitable symptomatic lesions.

  • Bleomycin is one sclerosant used in selected cases.

  • Multiple treatment sessions may sometimes be required.

  • Surgery has a role in selected lesions.

  • Treatment should be planned according to individual anatomy.

Causes, Symptoms & Diagnosis

Lymphatic malformations develop from abnormal formation of lymphatic channels, usually before birth. They may appear as a soft swelling or compressible lump and can cause pain, enlargement, skin changes, fluid leakage or recurrent infection.

Ultrasound/Doppler and MRI help identify the lesion and determine its extent.

Lymphatic Malformation Treatment

Treatment is individualised according to the lesion and symptoms.

Observation

Small, stable and minimally symptomatic lesions may be monitored with clinical and imaging follow-up.

Sclerotherapy

Image-guided sclerotherapy is a major treatment option for suitable symptomatic lymphatic malformations. A needle is placed into the abnormal cystic spaces under imaging guidance, fluid may be aspirated, and a sclerosant is introduced to reduce the abnormal spaces.

Bleomycin Sclerotherapy

Bleomycin can be used as a sclerosant for selected lymphatic malformations. It is particularly considered in appropriate cystic lesions after assessment of their anatomy and extent. At Citi Vascular Centre, bleomycin is available as part of selected vascular-malformation treatments.

Other sclerosants may also be considered depending on the lesion and treatment plan.

Surgery

Surgical excision may be considered for selected localized lesions, particularly when useful removal is technically feasible. Surgery can be more challenging when the malformation surrounds nerves, vessels or other important structures.

Combined Treatment

Complex lymphatic malformations may require staged or combined treatment, such as sclerotherapy with surgery for residual disease.

Sclerotherapy

Surgery

Minimally invasive

Requires surgical excision

Image-guided injection

Direct removal of accessible tissue

Common option for suitable symptomatic lesions

Selected localized lesions

Can treat cystic components

May address residual/localized disease

Multiple sessions may be needed

One operation may be sufficient in selected cases

Useful when open surgery is difficult

More challenging for infiltrative lesions

The appropriate approach depends on the lesion's type, location, depth and relationship with surrounding structures.

Treatment options for lymphatic malformation including sclerotherapy, Bleomycin injection, image-guided treatment, aspiration, laser therapy, selected surgical excision, and multidisciplinary care.
